Physical pain has always been part of human experience, and throughout history it is recorded that doctors and wise men and women have sought to ease pain. The attitudes of those suffering pain, however, have varied from stoical acceptance to sullen endurance. Today, most people consciously seek to avoid pain or to have their pain eased, although they do not always expect what in fact appears to be possible. This study of 13 patients with protracted pain was carried out at (...) The London Hospital by a professional group to see how patients regarded their own pain and the efforts of doctors and nurses to relieve it. Professor Duncan Vere lays before us the idealised guidelines used for recruiting volunteers on which to try and test new medicines. He points out that if these were followed rigidly, few, if any volunteers would be found for this vital work. Inducements are used, but the size of these determines whether society deems it right or wrong. However, the aim is to help and advise volunteers of the need for such tests and the risks involved and therefore the (...) information leaflet reprinted as part of the article indicates how the drug testers are attempting to encourage volunteers in as ethical a way as possible. This paper deals with four ethical issues in the development and application of business and management knowledge. The issues examined are: (1) failure to adopt or disclose knowledge with proven value that could benefit individuals, organizations, and society; (2) inappropriate implementation or incomplete disclosure of knowledge with proven potential; (3) use of knowledge for the exclusive benefit of a selected interest group even if harm is done to others; and (4) intentional falsification or misrepresentation of knowledge as something other than (...) what it is known to be. This paper was first presented at the London Medical Group's Annual Conference entitled Death: the last taboo held in February 1980. Dr Vere comments on the evidence of research done by him and his colleagues on the pain and discomfort suffered by patients who are dying and are in hospital. He contrasts this with the situation in hospices, analyses the differences, and attributes much of the unnecessary pain suffered in hospitals to attitudes of staff, as well as to a (...) reluctance by relatives to allow patients with terminal pain to die at home. He deprecates the tendency in hospitals to separate 'curing' and 'caring' and suggests that sometimes the best help involves 'doing' less, not more. (shrink)

In a recent Opinion article, Sui and Humphreys [1] argue that experimental findings suggest self is ‘special’, in that self-reference serves a binding function within human cognitive economy. Contrasting their view with other functionalist positions, chiefly Dennett's [2], they deny that self is a convenient fiction and adduce findings to show that a ‘core self representation’ serves as an ‘integrative glue’ helping to bind distinct types of information as well as distinct stages of psycho- logical processing. In other words, where (...) Dennett regards self as analogous to a center of gravity, a simplification posited by observers, Sui and Humphreys regard self as a function that modulates mental processes. In practice, however, the concept of ‘self’ they employ is not unlike Dennett's. We side with Sui and Humphreys in hold- ing that self-reference modulates mental processes: reference to self during a task can bind memory to source, increase perceptual integration, and link attention to decision making, among other things. What is more, these functions are not reducible to other factors such as semantic coding, familiarity, or reward [3]. But whereas Sui and Humphreys contribute important empirical detail, the binding functions they describe are compatible with Dennett's version of functionalism, which treats self as an artifact of social process. This volume contains thirteen papers that were presented at the 2017 Annual Meeting of the Canadian Society for History and Philosophy of Mathematics/Société canadienne d’histoire et de philosophie des mathématiques, which was held at Ryerson University in Toronto. It showcases rigorously reviewed modern scholarship on an interesting variety of topics in the history and philosophy of mathematics from Ancient Greece to the twentieth century. A series of chapters all set in the eighteenth century consider topics such as John Marsh’s techniques (...) for the computation of decimal fractions, Euler’s efforts to compute the surface area of scalene cones, a little-known work by John Playfair on the practical aspects of mathematics, and Monge’s use of descriptive geometry. After a brief stop in the nineteenth century to consider the culture of research mathematics in 1860s Prussia, the book moves into the twentieth century with an examination of the historical context within which the Axiom of Choice was developed and a paper discussing Anatoly Vlasov’s adaptation of the Boltzmann equation to ionized gases. The remaining chapters deal with the philosophy of twentieth-century mathematics through topics such as an historically informed discussion of finitism and its limits; a reexamination of Mary Leng’s defenses of mathematical fictionalism through an alternative, anti-realist approach to mathematics; and a look at the reasons that mathematicians select specific problems to pursue. Written by leading scholars in the field, these papers are accessible to not only mathematicians and students of the history and philosophy of mathematics, but also anyone with a general interest in mathematics. Experiential evidence shows that pain is associated with common meanings. These include a meaning of threat or danger, which is experienced as immediately distressing or unpleasant; cognitive meanings, which are focused on the long-term consequences of having chronic pain; and existential meanings such as hopelessness, which are more about the person with chronic pain than the pain itself. This interdisciplinary book - the second in the three-volume Meanings of Pain series edited by Dr Simon van Rysewyk - aims to better (...) understand pain by describing experiences of pain and the meanings these experiences hold for the people living through them. The lived experiences of pain described here involve various types of chronic pain, including spinal pain, labour pain, rheumatic pain, diabetic peripheral neuropathic pain, fibromyalgia, complex regional pain syndrome, endometriosis-associated pain, and cancer-related pain. Two chapters provide narrative descriptions of pain, recounted and interpreted by people with pain. Language is important to understanding the meaning of pain since it is the primary tool human beings use to manipulate meaning. As discussed in the book, linguistic meaning may hold clues to understanding some pain-related experiences, including the stigmatisation of people with pain, the dynamics of patient-clinician communication, and other issues, such as relationships between pain, public policy and the law, and attempts to develop a taxonomy of pain that is meaningful for patients. Clinical implications are described in each chapter. This book is intended for people with pain, their family members or caregivers, clinicians, researchers, advocates, and policy makers. (shrink)

The dominant paradigm of experiments in the social and behavioral sciences views an experiment as a test of a theory, where the theory is assumed to generalize beyond the experiment's specific conditions. According to this view, which Alan Newell once characterized as “playing twenty questions with nature,” theory is advanced one experiment at a time, and the integration of disparate findings is assumed to happen via the scientific publishing process. In this article, we argue that the process of integration is (...) at best inefficient, and at worst it does not, in fact, occur. We further show that the challenge of integration cannot be adequately addressed by recently proposed reforms that focus on the reliability and replicability of individual findings, nor simply by conducting more or larger experiments. Rather, the problem arises from the imprecise nature of social and behavioral theories and, consequently, a lack of commensurability across experiments conducted under different conditions. Therefore, researchers must fundamentally rethink how they design experiments and how the experiments relate to theory. We specifically describe an alternative framework, integrative experiment design, which intrinsically promotes commensurability and continuous integration of knowledge. In this paradigm, researchers explicitly map the design space of possible experiments associated with a given research question, embracing many potentially relevant theories rather than focusing on just one. Researchers then iteratively generate theories and test them with experiments explicitly sampled from the design space, allowing results to be integrated across experiments. Given recent methodological and technological developments, we conclude that this approach is feasible and would generate more-reliable, more-cumulative empirical and theoretical knowledge than the current paradigm – and with far greater efficiency. Auden's dictum "poetry makes nothing happen" has an enduring currency in poetic criticism, quoted ad nauseam to support the view that poetic discourse must never fall subservient to political ends.1 Conventional wisdom would hold that Hugh MacDiarmid, a poet more often noted for his obstinate commitment to communism, patently failed to heed this dictum. Commentaries on the target article offer diverse perspectives on integrative experiment design. Our responses engage three themes: (1) Disputes of our characterization of the problem, (2) skepticism toward our proposed solution, and (3) endorsement of the solution, with accompanying discussions of its implementation in existing work and its potential for other domains. Collectively, the commentaries enhance our confidence in the promise and viability of integrative experiment design, while highlighting important considerations about how it is used.

Faced with the systemic persistence of immiseration, and given normative (...) morality's complicity in sustaining such systems of domination, what ethical and practical recourse do the oppressed have to liberate themselves? This is a question which runs throughout the book and motivates the problem of an 'insurrectionist ethics,' as coined by Leonard Harris: a field of ethics which takes up a position, as he writes in the foreword, "that is in, starts from, and sojourns to a different place" (p. xiii). It is a re-imagining of the ethical which departs from romanticized notions of agency and reason, often in ways that favor descriptions over normative or explanatory principles. This shift gives it a high degree of flexibility with respect to its approach, which nevertheless remains guided by a common "insurrectionist sentiment" that lends consistency to the works of the text's contributors: they all "favor the abolition of what creates non-being, living death and necro-being" (p. xi). The book is split into a total of five topical sections, each of which discusses a different facet of insurrectionist ethics: its conceptions and context, its ties to liberation theory throughout the Americas, applications and correctives for its theory, its relation to pragmatism, and its future. Bookending contributions from the editors unpack the meaning and definition of an insurrectionist ethics.In this review, I survey several chapters of the volume by discussing Scriven's and Carter's definitions and examining a few ways in which these definitions produce tensions in a few selected contributions. Doing so will highlight some important differences between [End Page 110] the authors' approaches to insurrectionist ethics while underscoring the consistency which commits them to abolition; ultimately, Insurrectionist Ethics is as much an invitation as it is an introduction to the tradition it espouses.Scriven's introductory chapter, "The Very Idea of Insurrectionist Ethics," exposes an insurrectionist ethic within the thought of David Walker, while Carter's conclusive essay, "Death by a Thousand Cuts: Insurrectionist Ethics in a Present less Oppressive than the Past," attempts to pin down a definition by analyzing its essential elements. Their difference in approach is not insignificant. What is revealed through Scriven's angle, for example, is that insurrectionist ethics is indeed a tradition—to work through the meaning of such an ethics is to lend historical weight to the lives of past and present insurrectionists and to declare solidarity with their actions at the level of valuation. A number of such figures circulate throughout the volume, including David Walker, Maria W. Stewart, Alain L. Locke, Ida B. Wells-Barnett, Tommy Curry, and Leonard Harris. With the exception of the latter-most, none of these figures make direct contributions to the collection (although their spirits animate it). Its status as a tradition means that insurrectionist ethics is not limited to the confines of this volume, itself more knot than archive: such a tradition reaches well beyond the text and invites its readers to extend its concepts and debates in all directions. It has not one past but several, and not one future but many.Furthermore, Scriven's essay points out that insurrectionist ethics is not simply an indictment of corrupt institutions on the basis of some moral framework or other—it is foremost an outright rejection of the alliance between customary morality and systems of oppression; it searches for an ethos that condones direct action against such systems, operating at a philosophical level to produce conceptual and theoretical weaponry that "incite resistance and demands action" (p. xiv). Yet, as the form of the volume itself demonstrates, the arguments and methods of reasoning deployed by insurrectionists (past and present) to condone such actions has tended to vary, sometimes drastically. Professor Knox’s translation is, as one would expect, excellent. Even so, understanding this text will present difficulties to anyone who is anything less than expert not only in the philosophy of Hegel, but in those of Kant, Fichte and, especially, Schelling, because Hegel’s philosophy in 1802–3 was still by no means fully-fledged. The result is that the usual difficulties of Hegelian terminology are compounded by the infusion of the terminology and philosophical programme of Schelling; one’s tendency to interpret the text (...) in the light of Hegel’s mature philosophy has to be tempered by the realization that what prima facie looks ‘Hegelian’ is not necessarily so. This makes the initial agonies of a Hegel text even more hellish than usual, and no translator’s notes, no Introduction, however good, can spare one these pains when one starts reading. Perhaps the translator might have been a bit more generous with bracketed or footnoted explanations, for example, of words like ‘divine’, ‘difference’, ‘indifference’, and even more usefully of words that do not look odd in the context, like ‘immediately’ or ‘science’. On the other hand, since in reading a Hegel text for the first time it is usually best to push on regardless, it may be a help not to have too much in the way of explanation of technicalities to hold one up. In this paper, I assess Duncan Pritchard’s defense of the “orthodox” view on epistemic normativity. On this view, termed “epistemic value T-monism” (EVTM), only true belief has final value. Pritchard discusses three influential objections to EVTM: the swamping problem, the goal of inquiry problem, and the trivial truths problem. I primarily focus on Pritchard’s defense of the trivial truths problem: truth cannot be the only final epistemic value because we value “trivial” truths less than “significant” truths. In response, Pritchard (...) appeals to epistemic virtue: the virtuous agent desires “substantial” truths, where “substantiality” is a matter of the non-luckiness of the true belief. Thus, what has substantial value, for Pritchard, is well-grounded true belief. Yet, I argue, this moves away from “orthodox” EVTM: this view fails to attribute final epistemic value to non-grounded, accidentally true belief. Aside from giving up EVTM altogether, I suggest this leads to either a revision or revolution. On the revision interpretation, EVTM was always imprecise: what orthodoxy always really cared about was epistemically grounded truth rather than truth simpliciter. On the revolutionary reading, what we really care about, contra orthodoxy, formation of belief in line with virtues rather than truth simpliciter. (shrink)

_341_ _Objectives: _In patients with multivessel disease both the detection of the culprit lesion and the exact allocation are important preconditions for sufficient treatment and improved outcome. In a vessel based approach the combination of quantitative coronary angiography and fractional flow reserve measured by a pressure wire should be advantageous compared to myocardial SPECT, as morphological and functional information is delivered simultaneously. Therefore our aim was to evaluate MS in the detection and allocation of hemodynamically significant stenoses obtained by the (...) combined gold standard QCA plus FFR in patients with multivessel disease. _Methods: _FFR and QCA of 95 vessels in 80 patients have been evaluated. Hemodynamically significant target lesions were defined as intermediate stenoses between 50 and 75% with an FFR. (shrink)

The honour of being the first to teach philosophy in Australia belongs to the Congregationalist minister Barzillai Quaife (1798–1873), in the 1850s, but teaching philosophy did not formally begin until the 1880s, with the establishment of universities (Grave 1984). -/- Two approaches have dominated Western philosophy in Australia: Idealism and materialism. Idealism was prevalent between the 1880s and the 1930s, but dissipated thereafter. It was particularly associated with the work of the first professional philosophers in Australia, such as Henry Laurie (...) (1837–1922), Francis Anderson (1858–1941), William Mitchell (1861–1962) (who rejected the label) and a second generation including W. R. Boyce Gibson (1869–1935). Idealism in Australia often reflected Kantian themes, together with the British, particularly Scottish, revival of interest in Hegel through the work of the ‘Absolute Idealists’ T. H. Green (1836–1882), F. H. Bradley (1846–1924) and Henry Jones (1852–1922), the latter of whom conducted a popular lecture tour of Australia (Boucher 1990). -/- A number of the early New Zealand philosophers, including Duncan MacGregor (1843–1906), William Salmond (1835–1917), and Francis W. Dunlop (1874–1932) were educated in the Idealist tradition and were influential in their communities, but produced relatively little. William Anderson (1889–1955), at Auckland, brother of John at Sydney, was the only New Zealand philosopher that seemed to retain Idealist views. -/- In Australia, materialism gained prominence through the work of John Anderson, who arrived in Australia in 1927, and continues to be influential. However, as Lewis and others have complained, this has the drawback of turning monadic properties into relations . 2 In this article, we shall show that the Lewisian has the resources to block the above argument while simultaneously respecting the adicity of the relevant property and the contingency of the predication without changing his theory of properties. However, our analysis reveals a hidden ambiguity in the Lewisian conception of properties, and on either disambiguation the resulting account of properties seems inadequate.2.
